Community Healthlink’s Adult Community Clinical Services (ACCS) program provides community supports for adults 18 years and older living with chronic mental health needs.
Community Healthlink’s Adult Outpatient Counseling Centers, located in Worcester, Leominster and Fitchburg, provide caring, professional, and integrated behavioral health services for individuals with behavioral health concerns, including individual, family or group psychotherapy, psychiatric consultation and medication management.
CHL’s Adult Mobile Crisis Intervention (AMCI) provides crisis assessment, intervention and stabilization services in homes and other community settings for adults 18 years and older.
CHL’s BUDD Day Program serves adults 18 years and older who experience psychosocial, emotional, or behavioral problems in their day or residential setting, or both. The program also works with individuals who have recently been discharged from the hospital and require a transition back to the community.
Chelsea Street Program is an 8-bed residential program. The expectation of the program is for its residents to learn basic skills so that they may transition more independently into a residence that best meets their needs.
CHL provides counseling, support, and comfort to more than 3,000 children, teens, and their families in Worcester and Central Massachusetts each year.
CHL C.A.R.E.S. Club (Create. Achieve. Respect. Elevate. Succeed.) Program offers middle school youth and their families supportive services in the school, home, and community, by providing mentoring, academic help, brief individual counseling, and psychoeducation groups.
Community Healthlink's Community Support Program (CSP) provides short-term, intensive outreach and care management services to support individuals at risk of repeated psychiatric hospitalizations and/or inpatient substance abuse treatment programs.

CHL's Developmental Disabilities programs serve adults 18 years and older in Worcester County. Clients are referred to CHL directly from the Department of Disability Services.
Community Healthlink’s Early Intervention (EI) staff provide family-centered, community-based services for infants, toddlers, and families. Children ages birth to three may be eligible for Early Intervention services if they have identified disabilities, or if typical development is at risk because of certain birth or environmental circumstances.
Faith House and Beryl's House are 25-bed, six-month residential Recovery Homes for women. The mission of Faith House and Beryl's House are to provide safe, supportive recovery communities that will promote a sense of empowerment, personal accountability and the life skills essential for women and their families to stabilize and lead substance free, productive lives.
CHL's Family Support and Training program is provided by Family Partners who are caregivers with experience raising a youth with special needs.
CHL Geriatric Services provides a comprehensive range of mental healthcare services to seniors with a focus on preserving dignity, minimizing decline, and enhancing quality of life.
The Granite Street Program is a DMH referred, 11-bed group home for adults with mental health concerns.
The Green House Program is a 30-bed residential program that provides a supportive, safe environment to adult men who may be experiencing homelessness with a diagnosis of substance abuse and/or mental illness.
Health Home Management (HHM) offers services to members eligible for the One Care program offered through Commonwealth Care Alliance (CCA). Members are between the ages of 21 and 64 and must be eligible for MassHealth and Medicare benefits with CCA to receive HHM care management services. Care Partners work with members to help coordinate and manage the members’ benefits to ensure they have access to services that meet their care needs. Care Partners work with members to identify informal support, community resource providers, medical providers and long-term services resources.

In 1985, the Homeless Outreach & Advocacy Project (HOAP) was established with a commitment to link the region’s homeless to a comprehensive array of services that would include screenings, assessments, counseling, health care, rehabilitation, and referral to other support systems.
In-Home Therapy (IHT) is a structured, consistent, strength-based therapeutic relationship between a licensed clinician and an individual under the age of 21 and their family for the purpose of treating the youth's behavioral health needs.
Community Healthlink’s inpatient detox program provides 24-hour nursing care and observation for clients that are detoxing from drugs and alcohol.
Community Healthlink operates the Worcester County Juvenile Court Clinic under an agreement with the Massachusetts Department of Mental Health and the Trial Court of Massachusetts.

The Intensive Medical Group Living Environment differs from other programs in that the program specializes in serving Persons with medical needs as well as Persons who require a significant amount of supervision of day to day tasks.
Motivating Youth Recovery (MYR) is a 12-bed acute detoxification and stabilization program for adolescents 13-17 years old with substance use disorders and other co-occurring disorders.
CHL's North County Supported Housing Program (NCSHP) supports individuals 18 years and older referred through the Continuum of Care, assisting with their housing, mental health and medical health needs and providing transportation and supporting the individuals' ability to live independently in the community.
The North Village Community Housing Program provides safe, permanent affordable housing for families in recovery from the disease of addiction, specifically families with one or both parents in recovery and who have custody of one or more children, or a reunification plan.
CHL's Office-Based Addiction Program allows individuals to work with medical and behavioral health providers in CHL outpatient clinics to help recover from opioid use.
The Orchard Street program is a 9-12 month residential treatment program that serves mothers pursuing recovery from addiction to drugs and their children. The program specializes in working with mothers and their children who have Department of Children and Families (DCF) involvement in order to reunify.
As a post-detox Clinical Stabilization Service (CSS) program, PASSages provides short-term (up to 30 days), intensive treatment for individuals in the early stages of substance abuse recovery.
Located in Worcester, Leominster and Fitchburg, CHL’s Program of Assertive Community Treatment (PACT) provides intensive community-based treatment for individuals living with a pervasive mental health diagnosis.
The Randolph Program's services include interventions and supports that manage psychiatric symptoms in the community, restore or maintain independent living in the community, restore or maintain daily living skills, promote wellness and the management of medical conditions and assist Persons to restore or maintain and use their strengths and skills to undertake employment
For over 30 years, Community Healthlink’s Recovery Environments and Supports has provided residential services and supports to clients utilizing a strength-based approach to provide a comprehensive continuum of services. These services are aimed at supporting clients in their recovery from addiction, persistent mental illness, homelessness, and the numerous associated stressors.
Respite Beds is a 24-hour staffed program that provides short-term, temporary housing and support services for those in transition from one living arrangement to another, sometimes from a hospital or community inpatient unit. Respite Beds is a program funded by the Department of Mental Health.
CHL's Safe Haven is a 13-bed transitional housing program for chronically homeless adults, dually diagnosed, who generally suffer from untreated mental illness and substance abuse issues.
Sagamore Road Program is a 6-bed residential recovery-focused treatment program. The expectation of the program is for its residents to learn basic recovery skills so that they may transition more independently to sober-living or a residence that best meets their needs.
CHL clinicians provide on-site individual counseling for children and adolescents in a school setting, as well as access to clinic-based psychiatry services as needed.
CHL's Stabilization Treatment and Recovery (STAR) program provides alternative shelter for individuals who are unable or unwilling to utilize traditional services/placement.
The S.T.E.P. Clinic at CHL provides clinical service to individuals between the ages of 18 and 40 who are experiencing early psychotic symptoms.
The Tenancy Preservation Program (TPP) provides a clinical presence in the Central Massachusetts Division of the Housing Court and offers short-term intensive case management to eligible households whose tenancy problems are related to a disability.
Community Healthlink’s Thayer Transitional Support Service (TSS) program is a step-down program that provides inpatient substance abuse treatment and transitional support.
The Together for Kids (TFK) program provides a comprehensive mental health promotion, prevention, and intervention model of service comprised of the following components: 1) infant, toddler, preschool, and school age (ages 5-13) behavioral health consultation for educators and families; 2) trainings for educators and families on a variety of topics related to social-emotional health in children and 3) referrals for more intensive services (e.g., child/parent psychotherapy, psychological assessment).
CHL’s Victim Services program provides psychotherapy and case management to children (ages 6 and older), adolescents and adults exposed to violence, homicide, sexual assault, stalking, harassment, LGBTQ victimization, disasters, and other traumatizing experiences.
Windsor Street House is an eight-bed Department of Mental Health (DMH) funded closed referral program, that serves adults who struggle with persistent mental illness.
