    Community Healthlink’s Adult Community Clinical Services (ACCS) program provides community supports for adults 18 years and older living with chronic mental health needs.

  • BUDD Day Program

    CHL’s BUDD Day Program serves adults 18 years and older who experience psychosocial, emotional, or behavioral problems in their day or residential setting, or both. The program also works with individuals who have recently been discharged from the hospital and require a transition back to the community.

    CHL C.A.R.E.S. Club (Create. Achieve. Respect. Elevate. Succeed.) Program offers middle school youth and their families supportive services in the school, home, and community, by providing mentoring, academic help, brief individual counseling, and psychoeducation groups.

    CHL's Developmental Disabilities programs serve adults 18 years and older in Worcester County. Clients are referred to CHL directly from the Department of Disability Services.

  • Early Intervention

    Community Healthlink’s Early Intervention (EI) staff provide family-centered, community-based services for infants, toddlers, and families. Children ages birth to three may be eligible for Early Intervention services if they have identified disabilities, or if typical development is at risk because of certain birth or environmental circumstances.

    Faith House and Beryl's House are 25-bed, six-month residential Recovery Homes for women. The mission of Faith House and Beryl's House are to provide safe, supportive recovery communities that will promote a sense of empowerment, personal accountability and the life skills essential for women and their families to stabilize and lead substance free, productive lives.

    The Green House Program is a 30-bed residential program that provides a supportive, safe environment to adult men who may be experiencing homelessness with a diagnosis of substance abuse and/or mental illness.

  • Health Home Management 

    Health Home Management (HHM) offers services to members eligible for the One Care program offered through Commonwealth Care Alliance (CCA).  Members are between the ages of 21 and 64 and must be eligible for MassHealth and Medicare benefits with CCA to receive HHM care management services. Care Partners work with members to help coordinate and manage the members’ benefits to ensure they have access to services that meet their care needs.  Care Partners work with members to identify informal support, community resource providers, medical providers and long-term services resources.

  • HOAP Housing Case Management

    In 1985, the Homeless Outreach & Advocacy Project (HOAP) was established with a commitment to link the region’s homeless to a comprehensive array of services that would include screenings, assessments, counseling, health care, rehabilitation, and referral to other support systems.

    In-Home Therapy (IHT) is a structured, consistent, strength-based therapeutic relationship between a licensed clinician and an individual under the age of 21 and their family for the purpose of treating the youth's behavioral health needs.

    The Orchard Street program is a 9-12 month residential treatment program that serves mothers pursuing recovery from addiction to drugs and their children.     Respite Beds is a 24-hour staffed program that provides short-term, temporary housing and support services for those in transition from one living arrangement to another, sometimes from a hospital or community inpatient unit.     The Tenancy Preservation Program (TPP) provides a clinical presence in the Central Massachusetts Division of the Housing Court and offers short-term intensive case management to eligible households whose tenancy problems are related to a disability.

  • Together for Kids

    The Together for Kids (TFK) program provides a comprehensive mental health promotion, prevention, and intervention model of service comprised of the following components: 1) infant, toddler, preschool, and school age (ages 5-13) behavioral health consultation for educators and families; 2) trainings for educators and families on a variety of topics related to social-emotional health in children and 3) referrals for more intensive services (e.g., child/parent psychotherapy, psychological assessment).
