Community Support Program

Community Healthlink's Community Support Program (CSP) provides short-term, intensive outreach and care management services to support individuals at risk of repeated psychiatric hospitalizations and or inpatient substance abuse treatment programs. The services are flexible and focus on assisting individuals encountering challenges accessing and or sustaining health and other social needs in the community. CSP team consists of outreach workers from diverse communities and backgrounds who speak the following languages: English, Spanish, Vietnamese, Khmer, Twi, and Albanian.
 
CSP Services include:
•    Coordination with health providers to improve care and enhance wellness CSP Signpost
•    Providing “hands on” case management support, including short term transportation to appointments with health care providers
•    Connecting clients to community resources: housing, food, benefits/entitlement programs
•    Linkages to peer support and self help groups

CSP is available to individuals with Mass Health who have behavioral health care managed by MBHP, Network, and/or Beacon Health.
